Healthcare Inequities and Discrimination: Transgender Surgeries Covered While Non-Transgender People Face Denial of Essential Care and Denying President Trump’s IVF Family-Building Executive Orders.

My name is Ryan Samsel, a J6er, as many know, who was severely beaten and tortured and left in a small cell no larger than a dog kennel. I was denied all the necessary surgeries recommended by major universities during my time of incarceration. Initially, I believed this was solely due to my imprisonment, a part of the torture tactic aimed at denying me these life-saving, quality-of-life surgeries. I entered the prison system completely healthy, with only a minor 2 mm lump in my chest.
I often prayed to God for my day of freedom, hoping for fair medical treatment upon my release. However, the government denies any wrongdoing on their part, claiming that none of my constitutional rights were violated. It seems to be their norm to zip-tie individuals, subject them to violence, disregard professional medical advice, and confine them in a closet—leaving the fate of lives unknown. I did not succumb; I survived until the end. I am proud to say I never bent the knee, nor was it ever a temptation.
My day of freedom has arrived, but not without great cost. I was released from prison on January 21, 2025, with nothing—no money, no food, no clothes, and nowhere to go. I was entirely on my own.

The first thing I did was seek assistance from the state welfare system. I had read that since my incarceration, I would be entitled to welfare food stamps and low or free medical care. However, I quickly realized this would not be an easy process. I was denied food stamps due to my pardon, as there was no record of my incarceration.
Although I was awarded welfare medical assistance, this is where the issues began. I consulted three different doctors, all of whom stated that I would need surgery on my face and mandible (jaw). Additionally, the once 2 mm lump had transformed into an 18 mm pre-cancerous vascular obstruction in my chest, preventing blood from returning to my heart. The government created these issues and now denies me the medical treatment necessary to resolve them.
Fortunately, I met a former Marine turned MD who informed me about how the healthcare system denies many individuals and restricts doctors from performing personalized surgical techniques. Sometimes, insurance companies even send agents into the operating room to ensure doctors follow their guidelines, despite the agents having little to no medical experience. Furthermore, many doctors are instructed to vaccinate patients while they are under anesthesia. Many ethical doctors are leaving the United States to practice medicine in pop-up clinics in Middle Eastern countries, where they can provide ethical care and treat each patient individually. Although humans are anatomically similar, we are each unique, and a standardized surgical approach is not always the answer, which prevents doctors from doing what they do best.

I am one such patient who had to travel across the globe to obtain affordable medical care. I required facial reconstructive surgery with bone grafting and jaw alignment due to the brutal assault I suffered while in custody—without even being charged with a crime at the time. This assault was an attempt to coerce me into betraying my beloved country. The FBI often offered me a way out, suggesting that if I testified against President Donald Trump and other January Sixers, I would receive the medical care I needed.
I am not the only one being denied medical care. This issue appears to be a new phenomenon stemming from the Affordable Care Act. Upon returning from the Middle East, I still had two more surgeries to get: first, a Glansectomy to remove the obstruction, followed by a balloon angioplasty to open up the blood vessels returning blood to my heart. However, I was quickly denied.
Upon reviewing my insurance policy, I discovered that while plastic reconstructive surgery, gender-affirming surgery, voice therapy, eyebrow lifts, breast surgeries, and Botox were covered, my reconstructive surgery was not. When I asked my insurance agent about this, he inquired about my gender identity. I mistakenly identified as male.
These surgeries are only covered by taxpayer dollars if one has gender dysphoria; however, individuals with serious medical issues often face significant barriers. Many may find themselves in a position similar to mine, needing to search for a trustworthy doctor who still holds ethical standards and can provide personalized care. This is not an isolated issue affecting just J6ers; many people are trying to start families or require basic medical care that they simply cannot afford because government funds are often allocated to controversial procedures.
Donald Trump recently stated that the government and insurance companies would cover egg freezing for females. Just recently, Raechal Genco, another J6 defendant, inquired about freezing her eggs to someday have a family. The insurance company through her job at Penn Medicine quickly denied her request, stating they do not cover IVF, which costs approximately $15,000. I have sacrificed four years of my life, and I came home with the hope of receiving medical treatment and starting a family.

The truth is, the average person today is under attack. If you seek to change your sex, receive an eyebrow lift, or undergo speech therapy to speak like the opposite sex, you will be fully approved for coverage. My next two surgeries, including travel, will cost approximately $15,000, and many couples’ hopes of having a family are being crushed.
Denying the opportunity to create life is both unfair and heartbreaking. It costs nearly $50,000 in taxpayer dollars to change one’s sex, an additional $20,000 for aftercare, totaling around $70,000. Meanwhile, I struggle to put food on the table, maintain a roof over my head, and receive basic medical care—all while hoping to someday have a family.
Once I regain my footing, I hope to have a family. However, because insurance companies and state governments defy the president’s executive orders, not only will my dream of being a proud father be compromised, but many people like Raechal Genco will never have the chance to be a mother. The truth is, we are being discriminated against.
So please, I urge you to help us reclaim our lives. If President Trump sees this, I beg you—time is running out for us. Please help us.
Many J6ers have given far more than just four years of their lives. If the government can fund surgeries for gender transition, they should also ensure access to basic medical care and the opportunity to have children for all individuals.
